1) Is your DR plan documented and in printed form at more than one location and distributed to key personnel?

2) Are you going beyond data backups and off site replication as part of your DR plan?

3) Are all software installation files and patches on physical media in an offsite location?

4) Are all your software licenses and installation keys documented?

5) Are all system dependencies along with the required system startup order documented and diagramed?

6) Are your data recovery processes tested on a regular basis?

7) Are you testing the recovery of large amounts of data and including applications such as Exchange, MSSQL or Oracle?

8) As your environment changes, is the DR plan updated?

9) Are chain of command and communication directives clearly defined?

10) Do you have a way to immediately notify all your employees with instructions on what to do in the event of an office closure or other event?

11) Are service level agreements defined for business units and the end user community?

12) Has end user connectivity been addressed as part of the DR plan?
